Supporting Collaborative Pregnancy Planning

Pregnancy and postpartum planning conversations can feel overwhelming — especially for individuals navigating substance use, recovery, mental health challenges, or complex care systems.

This toolkit was developed to support respectful, patient-centered communication and shared decision-making between patients, families, and care teams.

The goal is not to direct decisions for patients, but to create safer, more collaborative planning conversations grounded in trust, transparency, and individualized care.
